A dental membership plan is a direct agreement between you and our Glendale office -- no insurance company in the middle. You pay a flat annual fee, receive the included services, and get a member discount on any additional treatment. No claims forms. No prior authorizations. No network restrictions. No one deciding what care you can have.
These plans are built for patients who are uninsured, underinsured, on Medicare (which excludes dental), or who simply find that annual premiums do not match what they actually get back. A typical Arizona dental insurance plan costs $240 to $600 per year, carries a $50 to $100 deductible, imposes a 6 to 12 month waiting period before covering restorative work, and caps benefits at $1,000 to $1,500. Our Essential Care plan covers your two annual cleanings, two exams, and X-rays for $229 per year -- with 15% off any treatment beyond that and no cap on savings.

Two professional cleanings, two comprehensive exams, and two X-ray visits at non-member rates typically total $600–750. Essential Care covers all of it for $229 -- plus 15% off any scheduled treatment during your plan year. New patients who enroll at their first visit get this same rate.

No. Our membership plans are not insurance. They are direct-pay annual agreements between you and our Glendale, AZ office. You pay a flat fee, receive the included services, and get a member discount on any treatment beyond what is covered. There are no claims to file, no network restrictions, no prior authorizations, and no insurance regulations involved. Every dollar you pay goes directly to your care.
Coverage is active the day you enroll. There is no waiting period on any plan, including emergency coverage under Emergency Access. You can schedule your first cleaning or use your emergency benefits at the same appointment where you sign up.
Emergency Access covers unlimited emergency exams and X-rays for one full year at $149. There is no per-visit copay and no limit on the number of emergency visits during your plan year. If you later upgrade to Essential Care, Periodontal Support, or Premier Care, the $149 you paid is credited in full toward the higher plan.
Patients who enroll in Essential Care at their first visit -- including patients who completed our New Patient Special -- pay $229 per year, the same rate as existing members. New patients who enroll at a later appointment pay the standard new-member rate of $299 per year. There is no difference in benefits based on when you enroll.
Periodontal Support provides four periodontal maintenance visits per year, two comprehensive exams with full X-rays, one OraCare oral health kit per year, two fluoride or nano-hydroxyapatite treatments, and 15% off all additional treatment. It is designed for patients who are currently in active periodontal therapy or who require more frequent maintenance to keep gum disease stable.
Once per plan year, every Premier Care member selects one of three benefits: professional in-office teeth whitening at our Glendale office ($400+ value), a professional take-home whitening kit with custom trays and professional-grade gel, or a $200 credit toward any cosmetic, spa, or dental treatment. You can choose your benefit at enrollment or at any point during your plan year.
Each additional household member enrolls at the add-on rate for their chosen plan. Essential Care family add-ons are $149 per adult or $99 per child per year. Periodontal Support adds an adult for $299 per year. Premier Care adds an adult for $449 per year. The family discount applies to the lowest-priced membership in the household. Emergency Access does not currently offer a family add-on rate.
Included preventive services are listed for each plan. Implants, crowns, veneers, and other restorative or cosmetic procedures are not part of the flat annual fee, but all members receive a treatment discount with no annual cap -- 15% for Essential Care and Periodontal Support members, and 20% for Premier Care members. There is no limit on how much you can save using your member rate.
Plans are annual with no long-term contract. If you need to cancel, we refund the pro-rated unused portion of your plan fee, minus the full non-member rate for any services you have already received during the plan year.
Our membership plans are designed for patients who pay directly and do not have active dental insurance, or who choose not to use it. If you have active dental insurance, we will bill your insurance directly for covered services. The member discount does not apply to the same service that insurance is billed for.
