Signs You Need One

When a root canal is necessary

A root canal becomes necessary when the pulp -- the nerve tissue inside your tooth -- becomes infected or inflamed beyond the point of recovery. These are the most common signs.

Severe Spontaneous Pain

Throbbing pain that comes on without any trigger -- especially at night -- is a hallmark sign of pulp infection. The nerve is inflamed and under pressure inside the tooth. This does not go away on its own.

Pain When Biting or Chewing

Sharp pain every time you bite down on a tooth indicates infection has spread to the ligament surrounding the root. The tooth is telling you the infection has moved beyond the pulp chamber.

Prolonged Sensitivity to Hot or Cold

Lingering sensitivity -- pain that continues 30 seconds or more after a hot or cold stimulus is removed -- signals that the pulp is dying or already dead. Normal teeth do not stay sensitive this long.

Dental Abscess or Facial Swelling

Swelling in the gum, jaw, or face means the infection has spread outside the tooth. A dental abscess can be dangerous if it spreads to the jaw or airway. This requires immediate treatment -- do not wait.

Darkening or Discolored Tooth

A tooth that has turned gray, brown, or noticeably darker than its neighbors has often lost its blood supply. Pulp necrosis can be painless at first, but the infection is still present and will eventually cause problems.

Pimple on the Gum

A recurring small bump or blister on the gum near a tooth is a sinus tract -- a drainage channel created by the infection. It may come and go as pressure builds and releases, but the infection at the root is still active.

Pain Management
Managing pain while you wait

Ibuprofen (if you can take NSAIDs) is generally more effective than acetaminophen for dental infection pain. Apply a cold pack to your cheek to reduce swelling. Avoid heat on the affected side -- it can intensify throbbing. Avoid alcohol, which can interfere with anesthesia.

Don't Delay
Tooth infections can spread rapidly

Antibiotics prescribed elsewhere can temporarily suppress symptoms but do not eliminate the infection. The source is inside the tooth, which antibiotics cannot fully penetrate. Without root canal treatment, infection can spread to the jaw, neck, or in serious cases the airway -- a life-threatening emergency.

If Sedation is Planned
IV sedation requires preparation

Do not eat or drink for at least 6 hours before your appointment (small amounts of water are usually fine up to 2 hours prior). Arrange for a driver -- you cannot drive after IV sedation. Wear comfortable, loose-sleeved clothing for IV access. We will review your medical history before proceeding.

A root canal should not hurt more than getting a filling. We thoroughly numb the tooth and surrounding tissue before we begin. Most patients are surprised by how comfortable the procedure is compared to the tooth pain that brought them in. If you are anxious or have a high sensitivity to dental procedures, IV sedation is available so you feel nothing and remember nothing.
Most single-canal teeth -- front teeth and premolars -- can be completed in 60 to 90 minutes. Molars have more canals and typically take 90 to 120 minutes. When we place a CEREC crown the same day, plan for an additional 30 to 45 minutes for the digital scan, milling, and crown fitting. We will give you a realistic time estimate once we review your X-rays.
Yes. We offer IV sedation administered by our board-certified dental anesthesiologist. You are deeply relaxed throughout the entire procedure and have little to no memory of it afterward. This is ideal for patients with dental anxiety, a strong gag reflex, or anyone who wants to sleep through the appointment and wake up with the treatment complete.
In most cases, yes. Removing the pulp leaves the tooth more brittle and prone to fracture under biting force -- especially for back teeth. A crown protects the tooth and restores full function. We can mill a CEREC ceramic crown in-office the same day as your root canal so you leave with a permanent restoration rather than a fragile temporary.
Cost varies by tooth location. Front teeth start around $700 to $900; molars range from $900 to $1,300. A same-day CEREC crown adds to the total.
