A root canal becomes necessary when the pulp -- the nerve tissue inside your tooth -- becomes infected or inflamed beyond the point of recovery. These are the most common signs.
Throbbing pain that comes on without any trigger -- especially at night -- is a hallmark sign of pulp infection. The nerve is inflamed and under pressure inside the tooth. This does not go away on its own.
Sharp pain every time you bite down on a tooth indicates infection has spread to the ligament surrounding the root. The tooth is telling you the infection has moved beyond the pulp chamber.
Lingering sensitivity -- pain that continues 30 seconds or more after a hot or cold stimulus is removed -- signals that the pulp is dying or already dead. Normal teeth do not stay sensitive this long.
Swelling in the gum, jaw, or face means the infection has spread outside the tooth. A dental abscess can be dangerous if it spreads to the jaw or airway. This requires immediate treatment -- do not wait.
A tooth that has turned gray, brown, or noticeably darker than its neighbors has often lost its blood supply. Pulp necrosis can be painless at first, but the infection is still present and will eventually cause problems.
A recurring small bump or blister on the gum near a tooth is a sinus tract -- a drainage channel created by the infection. It may come and go as pressure builds and releases, but the infection at the root is still active.
A few things to keep in mind before you come in to ensure we can provide the safest and most effective treatment.
Ibuprofen (if you can take NSAIDs) is generally more effective than acetaminophen for dental infection pain. Apply a cold pack to your cheek to reduce swelling. Avoid heat on the affected side -- it can intensify throbbing. Avoid alcohol, which can interfere with anesthesia.
Antibiotics prescribed elsewhere can temporarily suppress symptoms but do not eliminate the infection. The source is inside the tooth, which antibiotics cannot fully penetrate. Without root canal treatment, infection can spread to the jaw, neck, or in serious cases the airway -- a life-threatening emergency.
Do not eat or drink for at least 6 hours before your appointment (small amounts of water are usually fine up to 2 hours prior). Arrange for a driver -- you cannot drive after IV sedation. Wear comfortable, loose-sleeved clothing for IV access. We will review your medical history before proceeding.
Modern root canal treatment is performed under local anesthesia and should feel no worse than getting a filling. Most of the pain associated with root canals is the infection beforehand, not the procedure itself. Treating the infection is what brings relief. Most patients leave the office more comfortable than when they arrived.
We hold appointment slots specifically for dental emergencies. When you call in pain, we make every effort to treat you the same day. Waiting a week with an infected tooth is not acceptable -- and it does not have to be your only option.
Most offices send you home with a temporary crown and schedule you back in two weeks. We mill your permanent CEREC ceramic crown in-office in about 30 minutes. You leave with a finished, permanent restoration the same day as your root canal.
Our board-certified dental anesthesiologist is on site for IV sedation cases. You are deeply relaxed for the entire procedure. This is the same level of anesthesia safety you would expect in a hospital setting, delivered in our comfortable Glendale office.
Many general dentists refer root canal cases to an endodontist, which means another appointment, another office, and more delays while you are in pain. We handle most root canals here in-house -- from diagnosis and treatment through same-day crown placement. Complex anatomy or retreatment cases that genuinely benefit from specialist care are referred directly.
"I was terrified going in -- I'd had a bad experience with a root canal years ago at another practice. They completely changed my expectations. I couldn't feel anything, they put my crown on the same day, and I was back at work the next morning. I genuinely can't believe how different this was."

An endodontist is a dental specialist who limits their practice to root canal procedures. A general dentist who performs root canals regularly uses the same instruments and techniques -- and also handles diagnosis, buildup, and crown placement so you never need to visit a second office.
At Smile Science, our Glendale dentists perform root canals in-house as a core part of emergency care. For the vast majority of patients -- including front teeth, premolars, and most molars -- you do not need a specialist referral. We evaluate, treat, and restore in a single visit without the delays of a specialist queue.
When we do refer: Cases with unusual canal anatomy, severely calcified canals, or retreatment of a prior root canal may be better served by a specialist. When that is the case, we will tell you directly and help coordinate the referral. We would rather refer appropriately than overpromise.
Same-day root canal slots are limited. Call now -- we will fit you in today.
From diagnosis to same-day crown, we handle the full episode of care in one office. Most routine cases do not need a specialist referral. If anatomy, calcification, or retreatment makes specialist care the better choice, we coordinate it directly -- no waiting rooms, no two-week gaps while you are in pain.
We complete most root canals in a single appointment. We clean, shape, and seal all canals in one visit so you are not living with a temporary filling waiting for a follow-up. Treatment is complete before you leave.
Our in-office CEREC milling unit fabricates a custom ceramic crown from a digital scan of your tooth in about 30 minutes. No temporaries, no second appointments, no lab fees. The crown is cemented the same day as your root canal.
We use digital X-rays and when necessary CBCT cone beam imaging to map your tooth anatomy before treatment. Understanding the exact number, shape, and curvature of the root canals before we start leads to more thorough treatment and fewer complications.
Root canal cost depends on which tooth needs treatment, whether a core buildup is required before crown placement, your insurance coverage, and your financing plan. All prices below are for the root canal procedure alone -- buildup and CEREC crown needs are confirmed at your appointment before anything begins.
Incisors and canines. Single-rooted teeth with simpler canal anatomy and the most affordable root canal outcome.
Bicuspids. May have one or two roots. A common source of emergency pain from deep decay or fracture.
Back teeth with three or more roots. The most common emergency root canal case and the most thorough to treat.
