Failed Osseointegration

When an implant never fuses to the bone or integrates poorly, it becomes mobile and painful. We assess remaining bone volume with a new 3D CBCT, remove the failed fixture, graft the site if needed, and develop a re-placement plan with a timeline you can count on.

Ill-Fitting Prosthetics

A poorly designed arch causes bite problems, gum irritation, and accelerated bone loss. Even if the underlying implants are sound, a poorly fitted prosthesis is a failure. We evaluate whether a prosthetic revision alone can correct the problem or whether implant revision is also required.

Infection / Peri-Implantitis

Peri-implantitis destroys the bone around the implant if left untreated. Early-stage cases can often be managed with debridement and antimicrobial therapy. Advanced cases may require surgical intervention, bone grafting, or implant removal and site rehabilitation before re-placement.

Insufficient Bone

Patients told elsewhere they have "too little bone" for implants are often candidates for zygomatic implants or advanced grafting protocols. Dr. Barrett performs zygomatic cases at Smile Science for patients with severe maxillary bone loss who have run out of conventional options.

Clinical faculty at Implant Pathway, where he instructs full-arch surgical protocols to dentists across the country. He trained Dr. Dawson and continues to perform the most complex revision and zygomatic cases at Smile Science, including patients who have been turned away by multiple other providers due to severe bone loss.

When standard implants are not possible, zygomatic implants anchor into the cheekbone and allow immediate full-arch restoration without bone grafting. Dr. Barrett evaluates every complex revision case personally before a treatment plan is finalized.

Sometimes, yes. If the implant is mobile but the site still has adequate bone, removal, site grafting, and re-placement after healing is often successful. If osseointegration never occurred, early removal and a grafted healing period gives the best result. A 3D CBCT is required to assess bone volume, density, and proximity to anatomical structures before any revision plan is made.

The most common causes are insufficient bone volume or density at placement, peri-implantitis (bacterial infection around the implant), overloading during healing, systemic factors like uncontrolled diabetes or bisphosphonate use, and poor surgical technique. Some failures are prosthetic -- implants that integrated fine but were restored with an ill-fitting or poorly designed prosthesis.

Peri-implantitis is a bacterial infection of the tissue and bone surrounding an implant, similar to periodontitis around natural teeth. Early-stage cases can often be managed with debridement, antimicrobial therapy, and improved home care. Advanced cases may require surgical intervention, bone grafting, or in severe situations, implant removal followed by site rehabilitation and re-placement.

Zygomatic implants are extra-long implants that anchor into the zygomatic (cheek) bone rather than the jaw. They are used when severe bone loss in the upper jaw makes standard implant placement impossible. Dr. Barrett performs zygomatic implant cases at Smile Science for patients who have been told they have insufficient bone for conventional All-On-4.

Timeline depends on what is required. A prosthetic-only revision (ill-fitting crown or arch) can often be completed in 2–4 weeks. Cases requiring implant removal, bone grafting, and re-placement typically require 4–9 months total. We give you a specific timeline at your free assessment.

Our 10-year fixture warranty applies to implants we place, including replacement implants in revision cases. Implants placed by other providers are evaluated and documented but are not covered under our warranty unless we place a replacement. Written warranty terms are provided before treatment begins.

No. Smile Science is 100% owned by Dr. Dawson and Dr. Turke, two practicing dentists. There are no outside investors, no private equity involvement, and no management company directing clinical decisions. Both owners practice here full time. In a DSO or corporate chain, treatment recommendations can be influenced by investor returns, production quotas, or rotating providers who do not know your history.
