- The dental membership plans featured on this website are NOT INSURANCE. Plan members pay annual membership fees in exchange for access to discounts on certain identified dental services rendered by Smile Science, PLLC and its providers. Dental membership plans may not be used in conjunction with any dental insurance plan. Dental membership plans are not qualified health plans under the Affordable Care Act, and do not meet the minimum creditable coverage requirements under M.G.L.c. 111M and 956 CMR 5.00. Dental membership plans offered through Smile Science are administered by Smile Science PLLC, a private dental practice: 20118 N 67th Ave. Ste 308, Glendale, AZ 85308. The dental membership plans offered by Smile Science are not affiliated with or endorsed by any state insurance department. - No Double-Dipping on Discounts: The discounts offered through the Membership Plans do not apply to Invisalign treatment and may not be combined with any other discounts, promotions, offers, or service packages provided by Smile Science Dental Spa. Patients must choose either the Membership Plan discount or any applicable Other Discount, but not both. Smile Science Dental Spa reserves the right to modify, limit, or deny the application of the Membership Plan discount in cases where other discounts or promotions have been applied to the same treatment.
- Membership Plan Terms: The Membership Plan discounts apply only to the individual patient who is a member of the plan. The discount is non-transferable and cannot be shared with other individuals or used for any purpose other than treatment services provided by Smile Science Dental Spa.
- Exclusions: The Membership Plan discounts do not apply to the purchase of dental products, gift cards, or any non-treatment related items or services offered by Smile Science Dental Spa.
- Modification and Termination: Smile Science Dental Spa reserves the right to modify, suspend, or terminate the Membership Plans or any associated discounts, terms, or conditions at any time, without prior notice. Any changes to the Membership Plans will be communicated to members through the contact information provided during enrollment.
- Governing Law: The Membership Plans and these conditions are governed by and construed in accordance with the laws of the jurisdiction in which the Smile Science Dental Spa practice is located, without regard to its conflict of law provisions.
